Child Safety Assessment: Outcome Decisions

The outcome decisions focus on target issues and Safety Plans.

Target Issues

In your analysis, what are the target issues that appear to be contributing to the child safety concern?  What are you worried about? This helps to target assistance to the main issues of concern.

Are there any unmet safety and well-being needs of the child(ren)? Such as:

  • Physical needs
  • Social needs
  • Supervision needs
  • Emotional needs
  • Educational needs
  • Assessment needs
  • Counselling

Safety Plan

Construct a plan that addresses the identified targeted issues of concern, and meets any unmet needs. (See Safety Plan template)
